Deed of Gift- This form should be completed by a person wishing to give tangible personal property.
Deposit Form–This form should be completed by on-campus groups wishing to deposit funds into a WOU Foundation account. Click here for the Excel form (use when depositing large amounts of cash)
Fund Request Form- This form should be completed by on-campus groups wishing to open a new Foundation fund.
Fund Change Form- should be completed to make changes to an existing Foundation fund.
Fund Withdrawal Form (For Check Payments)- This form should be completed by on-campus groups wishing to withdraw funds from a WOU Foundation account.
Direct Deposit Authorization Form– This form is used to provide the Foundation with banking information for payments related to the Withdrawal Form. This form should be submitted physically to the Foundation to the attention of the Finance Manager or securely emailed via Gmail’s Confidential Mode to carrillon@wou.edu.
WOU Sub W9 – This form is used to provide legal identification for payee’s related to the Withdrawal Form. This form should be submitted physically to the Foundation to the attention of the Finance Manager or securely emailed via Gmail’s Confidential Mode to carrillon@wou.edu
Credit Card Purchase Request Form- This form should be completed by on-campus groups wishing to make a purchase using a Foundation credit card.
Invoice for Services Provided– This form should be completed by a person providing a service that will be paid for with WOU Foundation funds.
Payroll Deduction Form- This form should be completed by faculty and staff wishing to make a donation (or recurring donations) by payroll deduction.
Raffle Guidelines- This document explains rules and regulations for raffles to be held by on-campus groups.
Raffle Report Packet- This packet includes all forms, required and optional, for conducting an on-campus raffle.
Request for Credit/Debit Card Reader- This form should be used by campus groups wishing to check-out a PayPal reader.
