Make a Report

If you would like to make a sexual misconduct (sexual assault, harassment, stalking, relationship violence) report to the University and potentially proceed with an investigation, you can complete the form linked below.

For Responsible Employees

If you are a Responsible Employee making a report on behalf of someone else, you must include your name, the name of the survivor, and all of the details that you are aware of.

Reporting Process

After you complete the form linked above, it will be forwarded to the DEI/Title IX office, who will do outreach. Additionally, an Abby’s House advocate will be forwarded the information and will reach out to offer support and resources. You may ask any questions you have to either of these people who do outreach.

Western Oregon University’s Land Acknowledgement
Western Oregon University in Monmouth, OR is located within the traditional homelands of the Luckiamute Band of Kalapuya. Following the Willamette Valley Treaty of 1855 (Kalapuya etc. Treaty), Kalapuya people were forcibly removed to reservations in Western Oregon. Today, living descendants of these people are a part of the Confederated Tribes of Grand Ronde Community of Oregon and the Confederated Tribes of the Siletz Indians.
