New Students – Registering with DAS

Step 1 – Admission to Western Oregon University

Once you have been admitted to Western Oregon University and received your student ID (V-number), please:

  • Set up your WOU email account
  • Review DAS common accommodations
  • Review Student Rights and Responsibilities

Step 2 – Documentation Guidelines

Documentation and/or a meeting with DAS staff may be required to determine eligibility for accommodations.

Documentation can be submitted in the following ways:

  • Upload through the application center
  • Fax: 503-838-8721

  • In person during office hours: Student Success Center, room 105, is open Monday through Friday, 9am-5pm

Important: Documentation requirements vary depending on the type of accommodation requested.

  • Housing accommodations: Please review the housing accommodations webpage for specific guidelines and requirements. University Housing 
  • Academic accommodations (online or in-person): Choose one of the following options:

Option 1: Submit documentation such as an IEP, 504 Plan, or a letter on official letterhead from a qualified licensed provider who specializes in your disability. The documentation must include:

  • A specific diagnosis
  • Functional limitations in an academic setting
  • Recommended accommodations to reduce those limitations

Option 2: Have your qualified licensed professional complete the Disability Verification Form (be sure to provide them with your V-number).

Note: A qualified licensed provider must have appropriate expertise in the diagnosed disability and follow established professional standards.

 

Step 3 – Application Review and Determination

Once your application and any required documentation are received:

  • Your file will be reviewed by DAS staff
  • You will be notified via your WOU email if additional information or a meeting is needed

If approved, you will be invited to schedule a one-on-one Accommodations Orientation, where you will learn how to activate and manage your accommodations using AIM each term.

Each Term: Using Your Accommodations

To use your accommodations:

  • Submit your requests through AIM each term
  • Contact DAS if you have questions—we’re here to help

We also strongly encourage you to communicate with your instructors each term to ensure they have received and understand your Faculty Notification Letter (FNL).
