Tuition & Fee Refund Appeal Form
Students who feel there is a special circumstance that warrants consideration may appeal the refund policy by using this form. The circumstance must be extraordinary for the appeal to be considered. Please attach any supporting documentation. Requests received more than 90 days after the drop/withdrawal may not be considered. Note: If a student wishes to appeal the refund policy due to medical reasons, do not complete this request.
