Release of Information Form

All interactions and information shared between the office of Student Conduct and students are confidential and protected under FERPA. The best approach to get information is to ask the student directly. We understand that this might be frustrating and difficult. However, this is a time of growth and maturation for the student. If a situation arises that involves your student, and your student wishes for you to be made aware of information pertaining to this situation, your student must fill out a Release of Information form prior to the office of Student Conduct being allowed to share any information.

Western Oregon University’s Land Acknowledgement
Western Oregon University in Monmouth, OR is located within the traditional homelands of the Luckiamute Band of Kalapuya. Following the Willamette Valley Treaty of 1855 (Kalapuya etc. Treaty), Kalapuya people were forcibly removed to reservations in Western Oregon. Today, living descendants of these people are a part of the Confederated Tribes of Grand Ronde Community of Oregon and the Confederated Tribes of the Siletz Indians.
