Are you driving your personal vehicle on WOU business? Fill out the driver authorization form and this form: Private Auto Certificate of Insurance Form.

Return completed form(s) to Public Safety.

Are you driving a state van? You need to take the van safety test. Please visit the following website: Van Safety Course; then watch the corresponding video, take the online van safety test, and provide a copy of the certificate of completion to Public Safety.

Please allow at least five business days for completed forms to be processed.

Western Oregon University’s Land Acknowledgement
Western Oregon University in Monmouth, OR is located within the traditional homelands of the Luckiamute Band of Kalapuya. Following the Willamette Valley Treaty of 1855 (Kalapuya etc. Treaty), Kalapuya people were forcibly removed to reservations in Western Oregon. Today, living descendants of these people are a part of the Confederated Tribes of Grand Ronde Community of Oregon and the Confederated Tribes of the Siletz Indians.
