Personal Information Change

Name Change Information

A request to change your name in Western Oregon University’s computer system can be made in person or in writing at the Human Resources office (WOU students, faculty and staff) in the administration building. Complete the name change form.

To process the name change HR must see your new original Social Security card and make a copy for your file.

If you are an employee (faculty, staff, or student), federal regulations require employers to validate that the employee’s name on record exactly matches the name which appears on the individual’s social security card to ensure proper tax reporting. Thus, in order to reflect your name change on your personnel and payroll records, you must present your social security card which reflects your new name to the Human Resources Office.

If you have not requested a change of name with the Social Security Administration, please apply for a new social security card with the nearest Social Security Administration Office. (Note: You will need to take an original legal document reflecting your new name to support your request for the name change. Examples of accepted legal documents include a birth certificate, marriage license, divorce decree, etc.) Once you receive your new Social Security card, please bring it to the Human Resources Office. If you have any questions or concerns, please call the Human Resources Office at Ext. 88490.

Social Security Number Change Information

A request to change your social security number in Western Oregon University’s Computer System can be made in person at the Human Resources (Faculty and Staff) or Payroll Office (WOU Students). Both offices are located in the Administration Building.

If you are an employee (faculty, staff, or student), federal regulations require employers to validate that each employee’s social security number and name on record exactly matches the social security number and name which appear on the individual’s social security card to ensure proper tax reporting. Thus, in order to change your social security number you must present your social security card (or a photocopy if done by mail or fax) which reflects your correct social security number to the Human Resources or Payroll Office.

Western Oregon University’s Land Acknowledgement
Western Oregon University in Monmouth, OR is located within the traditional homelands of the Luckiamute Band of Kalapuya. Following the Willamette Valley Treaty of 1855 (Kalapuya etc. Treaty), Kalapuya people were forcibly removed to reservations in Western Oregon. Today, living descendants of these people are a part of the Confederated Tribes of Grand Ronde Community of Oregon and the Confederated Tribes of the Siletz Indians.
