Student Absence Notification Information
Purpose:
In the case of an unplanned, extenuating circumstance (e.g. physical/mental health matters, death of a loved one, legal obligations/jury duty, etc.) that will keep a student from attending class, the Student Absence Notification System (SANS) form allows the Student Success and Advising office to relay the information to faculty members in a timely manner. This notification serves as a courtesy notice and does not excuse the absence.
Students:
If you are missing class for an unplanned, extenuating circumstance (e.g. physical/mental health matters, death of a loved one, legal obligations/jury duty, etc.) please fill out and submit the form below. Once submitted, all of your faculty members will be notified of your absence. If you are unable to complete the form electronically, please call 503-838-8428 or email studentsuccess@wou.edu for assistance. Please note that you are responsible for communicating with your faculty members to determine if you may make up any work missed during your absence. Any decisions regarding make up work are up to your faculty member.

Please do not use this form if:
- You will be missing class due to pregnancy or childbirth. Please review this form (https://graduate.wou.edu/files/2015/12/Title-IX-Pregnancy-Parenting-Absence-Request-form.pdf) for instructions on that absence process.
- You are missing class for a university sanctioned event, such as an athlete traveling to a game. Your coach or event coordinator will contact your faculty regarding the absence in another format.
