The purpose of this form is to report instances of sex or gender-based misconduct and/or incidents of bias at Western Oregon University (WOU).
- Sex or Gender Based Misconduct (Title IX) – Use this form to report incidents such as, sexual assault, sex/gender harassment, sex/gender discrimination, sexual exploitation and indecent exposure, bullying/intimidation, domestic violence, dating violence, and stalking.
- Bias Incident Reporting - Use this form to report an incident of bias if it is an act of conduct, speech, or expression that targets an individual or group based on actual or perceived race/ethnicity, color, religion, sex, sexual orientation, gender identity or expression, national origin, marital status, age, expunged juvenile record, preformance of duty in a uniformed service, physical or mental disability, or any other characteristic protected by under law ("Protected Class Status").
Anonymous submissions are accepted.
- However, WOU's ability to follow up with the reporting party, investigate, respond, and/or provide remedies may be limited or not possible.
- If you are reporting in your role as a responsible employee, you may not report anonymously. Otherwise, all individuals may submit an anonymous report.
For those who provide an email or phone number, the reporting party should expect communication within one business day.
