Directory Information:<\/strong><\/h4>\nFERPA authorizes the University to make public disclosure at its discretion, upon request, of directory information.<\/span><\/p>\nThe following is directory information<\/strong> at WOU:<\/span><\/p>\n\n- Student\u2019s full name<\/span><\/li>\n
- Student\u2019s photograph<\/span><\/li>\n
- Place of birth<\/span><\/li>\n
- Local address and telephone number<\/span><\/li>\n
- Permanent address and telephone number<\/span><\/li>\n
- Billing address<\/span><\/li>\n
- Class level (freshman, sophomore, junior, senior, graduate student)<\/span><\/li>\n
- Degree, major, minor, concentration, or certificate being pursued<\/span><\/li>\n
- Dates of enrollment<\/span><\/li>\n
- Degrees, honors, & certificates applied for or awarded (& award date)<\/span><\/li>\n
- Earned credit hours<\/span><\/li>\n
- WOU Email Address (subject to restrictions under state law)<\/span><\/li>\n
- Enrollment status (full-time, half-time, less than half-time)<\/span><\/li>\n
- Whether a student has applied for graduation<\/span><\/li>\n
- Participation in official recognized activities and sports<\/span><\/li>\n
- Height & weight of athletic team members<\/span><\/li>\n
- Job title and dates of WOU student employment<\/span><\/li>\n
- Most recent educational institution attended<\/span><\/span><\/li>\n<\/ul>\n
Non-Directory Information<\/strong><\/h4>\nNon-directory information may not be disclosed without the express detailed\u00a0written consent<\/strong> of the student.\u00a0Non-directory information is any part of students’ education records not designated as “directory information”.<\/span><\/p>\nExamples of non-directory information<\/strong> are:<\/span><\/p>\n\n- Class schedule (course reference numbers, meeting times, locations)<\/span><\/li>\n
- Classes enrolled in<\/li>\n
- WOU student ID number (V#)<\/span><\/li>\n
- Social Security number<\/span><\/li>\n
- Date of birth<\/span><\/li>\n
- GPA or Grades<\/span><\/li>\n
- Transcripts (official or unofficial)<\/li>\n
- Academic standing or eligibility to return to school<\/span><\/li>\n
- Gender<\/span><\/li>\n
- Parent names or parent addresses<\/span><\/li>\n

- Unmet degree requirements for graduation<\/li>\n
- Accounts receivable balance<\/li>\n

- Residency status<\/li>\n

FERPA affords students the right to opt-out of the disclosure of directory information. Institutional authority to release student directory information without consent is nullified<\/strong> when a student opts-out of disclosure of their directory information.\u00a0Students that opt-out are identifiable in university systems by the designator “confidential”.<\/strong><\/p>\nAbsolutely no information<\/strong> may be provided to anyone \u2013 parents, relatives, friends, other students, or prospective employers \u2013 about a student that has elected confidentiality for their records. This includes if the individual is or was a student, has ever enrolled in classes, or has earned a degree at Western Oregon University.<\/span><\/p>\nOnce a student restricts the release of directory information, in order to conduct any business with the university, the student must be logged in to a secure university system (WOU email is consider one such system), appear in person\u00a0<\/span>with photo ID<\/strong>, or, via mail or fax, provide a\u00a0<\/span>notarized<\/strong>\u00a0written request for release containing the following<\/span>:<\/p>\n\n- What information is to be released\u00a0(be as specific as possible)<\/li>\n
- To whom the information is to be released\u00a0(name, address)<\/li>\n
- The purpose for which it is to be released\u00a0(application for a specific job or admission to a graduate program, for example)<\/li>\n
- Student signature and date signed<\/li>\n<\/ul>\n
Restricted Directory Information is made available only where an emergency is involved, at the direction of a court order, or to Western Oregon University staff and faculty with a legitimate educational need to know.<\/p>\n
To prevent the disclosure of your directory information.<\/p>\n
\n- Complete a\u00a0Prevent Disclosure of Directory Information<\/a><\/span> form<\/li>\n
- Submit the form in-person with picture identification or<\/strong> have the form notarized.<\/li>\n
- The form is submitted to the Office of the Registrar, Welcome Center, Room 145, 345 North Monmouth Ave, Monmouth, OR 97361<\/li>\n
- Photo identification or a notarized written request is required (and will be required for all future information requests).<\/li>\n<\/ol>\n
A request to place or remove the restriction is effective no later than two working days after it is received in the Office of the Registrar. Submission of a Records Confidentiality Request form does not affect directory information already published or released.<\/p>\n
The restriction of information is permanent<\/strong><\/span> until the student requests, in writing, that it be removed.
