E-Form Agreements

Electronic Form Agreements

E-Form Agreements are used in our AIM database to manage your agreements electronically. When you log into AIM, you may be prompted to digitally sign any pending E-Form Agreements.

Some agreements only need to be completed once, others appear in AIM each term. You can view your signed E-Form Agreements anytime in AIM.

Confidentiality

Release of Information

ODS records are considered confidential; Information Regarding Confidentiality and Privacy includes the ODS Release of Information (ROI). A completed ROI allows ODS to share certain information with a designated person or persons.

Note that records requests can take up to 2 weeks for processing.

Need to make a change?

Add or Alter Accommodation

If you would like to add or alter your currently approved accommodations, please complete the Add/Alter Approved Accommodations Form. Also note that, if your accommodation request is approved and new or adjusted accommodations are eligible for use from the approval date forward; newly added accommodations cannot be retroactive to prior terms or classes.
